What Should I Order When Ordering Medical Records For A Client Applying For LTC Insurance?

In order to gather the appropriate medical information to underwrite a case for
Long Term Care Insurance, it will be necessary to request the following items:

  1. All medical records, including chart notes, from the client's primary physician with special attention to complete physical examinations that detail functional capacity (i.e. ADLs) as well as physical findings.

  2. All medical records from any other physician from which the client is receiving medical care with special attention to complete physical examinations that detail functional capacity (i.e. ADLs) as well as physical findings.

  3. All medical records from any hospitalization within the last five (5) years.


Remember: You may have to request records from the primary care physician, specialists as well as hospitals
